The
Renal Biopsy
Requisition Form (44kb)
initiates the renal pathology order.
Specimen kits containing pre-filled
fixative vials and pre-printed pre-paid air bills are supplied
at no charge by contacting our pathology outreach customer
service personnel or
submitting a request via the customer service form.

questions not covered here.
A renal biopsy is a precious
specimen. Its miniscule size is deceptive because of the
large quantity of information it harbors, information vital to
understanding the etiology, treatment, and prognosis of your
patient. It is imperative, therefore, that this tissue be
biopsied and prepared correctly and expeditiously.
Native kidney biopsy:
1. Immediately after retrieving
the biopsy core(s), place one portion of tissue into each of
three vials as indicated below:
a. White top vial –
Michel’s – expiration date on vial - for
immunofluorescence.
b. Yellow top vial –
Formalin – no expiration date - for light microscopy.
c. Blue top vial -
Glutaraldehyde – expiration date on vial - for electron
microscopy.
2. Complete all requested
information on reverse side of this requisition form.
Extra information for clinical history and/or insurance may
be attached to requisition for customer convenience.
3. Please provide name, phone,
and fax number of nephrologist and pathologist to receive a
copy of the report.
4. Confirmation of specimen
receipt including name and phone number of LSUHSC-Shreveport
Renal Pathologist performing renal consultation will be
faxed to sending institution upon specimen arrival.
Open (wedge) biopsy:
1. Immediately after retrieving
the biopsy, cut three (3) thin slices from cortex and place
one slice in each of three vials as indicated below:
a. White top vial –
Michel’s – expiration date on vial - for
immunofluorescence.
b. Yellow top vial –
Formalin – no expiration date - for light microscopy.
c. Blue top vial -
Glutaraldehyde – expiration date on vial - for electron
microscopy.
2. Complete all requested
information on reverse side of this requisition form.

3. Please provide name, phone,
and fax number of nephrologist and pathologist to receive a
copy of the report.
4. Confirmation of specimen
receipt including name and phone number of LSUHSC-Shreveport
Renal Pathologist performing renal consultation will be
faxed to sending institution upon specimen arrival.
The
clinician is called directly, the day the biopsy is received,
with the results of the light microscopy and immunofluorescent
microscopy.
Electron
microscopy is completed within 24 hours.
A complete
report is faxed immediately upon completion of the electron
microscopy.
Digital
images are hosted, e-mailed, or a hard copy sent dependent upon
the preference of the referring physician.
